The following documents must be submitted to the Admissions Committee:

– copy of a document (one of the documents) certifying the person provided for by the Law of Ukraine “On the Unified State Demographic Register and documents confirming the citizenship of Ukraine, certifying the person or his/her special status”;

– copy of the residence registration certificate (as an addition to ID-card);

– copy of the Taxpayer Identification Number (Individual tax ID certificate);

– copy of the military registration document (except cases stipulated by the law);

– state standard document certifying previously obtained educational (educational-qualification) level, which allows the admission, and an appendix to it;

– External independent evaluation certificate (certificates) in the relevant competitive subjects for 2018, 2019, 2020 and 2021;

– information card as an appendix to the certificate (certificates) of external independent evaluation displaying results of the tests;

– documents confirming the applicant’s right to enroll by interview, to participate in the competition based on the results of entrance exams and/or quota;

– in case of changing surname, name or patronymic – a copy of the document indicating the change of passport data should be submitted;

– four 3 x 4 cm color photos.
